What personal information is collected
We collect relevant data on the characteristics of the people we serve, such as age, sex, and disability, to improve service delivery. This data is collected at the beginning of services, at appropriate intervals, and at the end of services. Personal information may also be collected through intake forms, referral information, and other relevant assessments.
How personal information is used
Personal information is used to design and deliver individualized services that focus on the person’s strengths, abilities, needs, and preferences. We use this information to develop coordinated individualized service plans and to track progress toward established goals. Data analysis is also conducted to identify trends and areas needing performance improvement in our services and business functions.
Who personal information is shared with
Any release of confidential information is authorized by the person served or their legal representative through a signed “Authorization for Transportation and Release of Information” form. This consent is limited to the specific information identified and has a time limitation. With the person’s permission, reports and information are shared with advocates and other necessary staff involved in their case. We also adhere to the reporting requirements of our funding sources, such as Opportunities for Ohioans with Disabilities (OOD) and the Department of Developmental Disabilities (DoDD). SMS/mobile consent is not shared with third parties or affiliates.
Data Security and Storage
All records of the people we serve are kept in a secured electronic format within the Consumer Information drive and the SETWorks system. Physical records are securely stored in locked offices when not in use. Our electronic devices are protected with individual employee logins to prevent unauthorized access. We use cloud technology that backs up files as they are saved. Hard drives are removed and kept, and physical destruction of hard drives is performed by a certified third-party recycler when equipment is decommissioned.
